AICE chicago emergency grant application

This is the application form for the AICE Chicago Emergency Grant.

Chicago union members suffering from recent hardship may apply to receive this benefit.

This is a one-time payment per year of up to $500.

Areas of need may include but are not limited to: Injury or illness requiring hospitalization (urgent care included); loss of shelter; job loss; other (explanation requested)

When filled out, this form will be sent to the Executive Board, the group authorized to disburse payment. 

Applicants will receive a written response and explanation, including the possibility that they will be asked to provide more information. 

Approved payments will then be allocated within 30 days to the recipient through the AICE account.
